Association Board Members & Coaches are required to complete the following;
1. Background Live Scan Click Here, only if you are a new coach. Please bring a copy of your receipt when you attend the SYF Clinic.
2. Heads Up Concussion Training  Please remember to print a copy of your certificate with your Association's name at the top right and bring with you when you attend the SYF Clinic. 
3. Attend ONE SYF Clinic and have photo taken.  Dates will be on Master Calendar. 

Participant Certification Requirement:

  1. Certify for one team only

  2. Current league player card (typed) W/ Photo digitally inserted.

  3. Signed legal guardians consent

  4. Medical Clearance  (dated after Apr, 1st of current year) 

  5. Signed code of conduct, all parents / players 

  6. Proof of Age 

    1.  School Enrollment form required for all ages, must include address and Date of birth, on school letterhead or school stamped and signed.

    2. In the event where guardian is unable to get school enrollment form then participants may use original or certified copy of birth certificate, Military I.D Card, Passport, or school of enrollment form for proof of age.

    3. NO PHOTO COPIES allowed to be used for eligibility!!

  7. Valid residential address (current) Within teams H.S recruiting boundary

  8. SYF Waiver and release signed by parents or legal guardians 

  9. AB2007 Concussion awareness form signed by all participants and legal guardians.

  10. SIGNED 2019 CUETA

Instructions on how to run player/cheer cards/official rosters:
First you need to Run the chuck Norris report within reports in Blue Sombrero. 
Then Save the reports of player/cheer info by teams on your desktop 

Close all documents and then
Open up player/cheer card word doc and then following directions below; 

Go to tools
Select Mail merge manager then follow the steps; 
1. Select create new
  • Select form letters
2. Select recipients list
  • Select get list 
  •  Select open data source (this is the Saved reports of player/cheer info by teams on your desktop)
  •  Select document saved 
3. Insert placeholders onto the word document

Continue to follow the steps through complete merge

You will then place the digital photo of the player/cheerleader on the appropriate player/cheer card.
